Family Survey Project
Thanks for your interest in helping us with this evaluation project. We are interested in having parents of young children (grades 1-3) complete two quick online surveys. The surveys will only take 5 - 10 minutes each and by completing it you will be eligible for one of 4, $50 HyVee Gift Cards.
